Mechanism
Shrinks sebaceous glands, normalises follicular keratinisation, reduces C. acnes colonisation and damps inflammation — the only agent that hits all four acne drivers at once.

The full read
Decades of trials put long-term remission around 70–80% after a full cumulative dose, which nothing else in dermatology approaches. It is also the entry in this library with the most serious prescribing constraints: pregnancy exposure causes severe birth defects, which is why pregnancy-prevention programmes exist; liver enzymes and lipids are monitored; the mood-effect signal remains debated and is taken seriously regardless. Dryness, chapping and an initial flare are near-universal and manageable. For the facial-aesthetic question specifically, the downstream win is scarring you never get — active inflammatory acne left long enough leaves permanent texture that is far harder to treat than the acne was.
