An RCT found 82% of the microneedling+minoxidil group achieved >50 new hairs/cm² versus 4.5% with minoxidil alone. Use a 1.0–1.5mm needle weekly with proper sterilization, and don't apply minoxidil immediately after needling (absorption spike → systemic effects). One of the few adjuncts with genuinely strong data.
