Large RCTs show 5% topical minoxidil significantly increases hair count versus placebo at 48 weeks. It does not address the hormonal cause (DHT), so it's often combined with finasteride for men. Response takes 4–6 months and reverses if discontinued. Also has B-grade evidence when microneedling is added.
