Mechanism
A horizontal osteotomy through the mandibular symphysis frees the chin segment, which is advanced (or lengthened, shortened or rotated) and fixed with plates. Soft tissue follows at roughly 0.8–0.9 of the bony movement.

The full read
For genuine chin deficiency this outperforms both filler and implants on durability and on what it can correct: vertical height and asymmetry move too, which an implant cannot address. It also avoids the implant-specific risks of migration, infection and the bone resorption seen under some long-standing alloplastic chins. Costs are real — intraoral surgery, a period of numbness from mental nerve retraction that is usually temporary and occasionally is not, and a recovery measured in weeks. It is frequently planned alongside orthodontics or `bimax-surgery` when the whole lower face is retruded, which is a conversation worth having before treating the chin in isolation.
