Imidazole antifungal with weak local anti-androgen activity; also reduces Malassezia-driven scalp inflammation, which independently worsens shedding in seborrhoeic dermatitis.

The full read
The famous small study comparing 2% ketoconazole against minoxidil found comparable improvements in hair diameter, and it has never been replicated at scale — so treat it as suggestive, not established. What is solid is the seborrhoeic dermatitis half: an inflamed, flaking scalp sheds more, and controlling that helps whether or not the anti-androgen story holds. Practically, two to three washes a week with a few minutes of contact time, as a third item alongside `finasteride` and `minoxidil`. Cheap, low-risk, and not a substitute for either.
